Turned out it wasn't an issue with the phone. The extension config on asterisk somehow got currupted and did not allow call waiting on any phone for that extension. Deleting the extension and re-adding...

I report a bug, I get a response that Polycom is aware of the bug, and I have to go to another company to report the bug so it can be fixed? If I told that to my clients, I would lose them as a client.
